Consent for my child(ren) to participate in art classes and activities and assumption of the risk: Art activities and materials may include, but are not limited to paints, glues, pastels, chalk pastels, pencils, wood/plaster tools, fiber (yarn/fabric/papers), wax, plastic bags, scissors, plaster. Splatters Art Studio takes precautions to reduce risk and provide safe, healthy, and enjoyable experiences. I acknowledge that risks from participation in class activities and use of materials exist and I consent to have my child attend art class knowing these risks and their possible consequences including personal injury and damage to personal property. I understand that the safety and well-being of my child(ren) is a top priority at Splatters Art Studio. I understand and agree that Splatters Art Studio, Amy Marson & Staff will not be responsible for any injuries to my child(ren) or damage to my child(ren)’s property or art projects. Splatters Art Studio reserves the right to require parents or guardians to withdraw a child from class for disruptive behavior at any time. If I permit my child(ren) to play on equipment in the backyard or yard and my child(ren) incur any injury while on playground equipment including, but not limited to, Mari-go-round, Slide structure, ride on toys, see-saw, I agree that I assume all liability for any resulting injuries suffered by my child(ren) and that Splatters Art Studio, Amy Marson & staff assume no responsibility or liability whatsoever for such injuries. If your child has an emergency during our workshop and requires emergency medical attention, we will call 911, then the parent/Guardian, if they are unable to answer we will call the emergency contact. If child has accident or needs help in the washroom you give consent for Amy to attend to child to help them with their needs. I am responsible for my child(ren)’s art project once it leaves the studio. Splatters Art Studio, Amy Marson & staff are not responsible if the project causes any injury to child or if the art project is damaged once it has left the Studio. Waiver I hereby release, indemnify, hold harmless and forever discharge Splatters Art Studio, Amy Marson & staff and their heirs and assigns from all manner of actions, claims, or demands of whatever nature or kind that I or my child(ren) ever had, or can, shall or may have by reason of any and all damages, injuries, claims and other liability arising as a result of or in any way arising out of my child(ren)’s attendance and participation in any of the services offered by Splatters Art Studio, Amy Marson & staff. I further release, indemnify, hold harmless and forever discharge Splatters Art Studio, Amy Marson and their heirs and assigns from all manner of actions, claims, or demands of whatever nature or kind that I or my child(ren) ever had, or can, shall or may have by reason of any and all damages, injuries, claims and other liability arising as a result of or in any way arising out of my child(ren)’s use of the playground equipment including, but not limited to, slide structure, rideo on toys, mud kitchen
